Medical Records Office

The medical records office at Carson Valley Health is located on the 3rd floor of our Ironwood Medical Center building, 897 Ironwood Drive in Minden. Enter through the door facing Ironwood Drive and use the stairs or elevator to reach the 3rd floor.

Office Hours are:
Monday – Thursday 8AM – 5PM
Friday 8AM – 4PM

Requests for medical records:

All requests for medical records require a medical release authorization form that can be obtained from the medical records office during business hours or can be downloaded from our website.

Forms can be mailed to:

or faxed to: 775.782.1515.

All requests for patient health information must be authorized by patient’s signature or patient’s representative. A copy of your drivers license or another valid form of identification is required.

Please allow five (5) business days for your request to be processed.
